Use of Photographs

I consent to the use of photographs of my athlete(s) for use on the Brentwood SeaWolves website and/or social or print media for the purpose of increasing community awareness and participation on the team. If I do not agree, then I will send an email to [email protected] with such notification. *

Waiver and Release of Liability, Liberty Union High School District & City of Brentwood

In consideration of being allowed to participate in the activity of swimming and diving, I agree to release Liberty Union High Schoo District (LUHSD) and City of Brentwood and/or any employee from negligence to the fullest extent permitted by the law, for any complications or injury that may result to my child(ren), to me or my family while at LUHSD or City of Brentwood Facilities. I understand swimming and diving can be dangerous, and that a full range of known and unknown possible injuries can occur. This includes but is not limited to permanent disability and death.

I/We accept liability for any financial obligations my/our child may incur due to injury. In addition, I/we do, hereby waive, release and agree to hold harmless the Brentwood SeaWolves, the organizers, for any claim arising from an injury except to the extent and in the amount covered by accident or liability insurance.
